Quick Form

The Quick Form is used to collect information from site visitors. Forms can be used for many different things including public works complaints, contacting your office, information requests and more. Click on "Configure Your Form" to start building your form.


edit your form

  • Quick Form - Fields - This is the area where each of your form elements (text fields and checkboxes) will display after you create them. To create a new form element either click on the green "Add Form Element" button or select "Add a text field" or "Add checkboxes" from the left side menu.
  • Re-Order form elements - This is where you can re-arrange the order of the form elements that have been added.

new text field

  • Label - The label will appear next to the empty text field. Example labels include "Name", "Address", etc.
  • Text Box Type - Choose single line or multi line. All of the fields seen in this image are single line fields. A multi line field is larger and has more rows.
  • Field is required - Check this box if you would like this field to be required in order to submit the form.
  • Field Type - Choose one option. The field will format itself to best fit the type you choose.
  • Field Width - You can manually shrink the field width with this option. The default length is 30.
  • Default Value - Any text you put in this field will appear in the field on the form. For example, if the label for this field is "Name", you can enter "Please enter your name" in the Default Value field. Then, when someone is filling out your form, they will see the label "Name" next to a field that has the text "Please enter your name". They can then click on that field, the Default Value will disappear, and they can enter their name.

  • Label - The label will appear next to the empty text field. Example labels include "Name", "Address", etc.
  • Field is required - Check this box if you would like this field to be required in order to submit the form.
  • Field is numeric - Check this box if the data being collected is numeric.
  • Export each item in its own column - Check this box to control the formatting of how the data is sent to you.
  • Add/edit options - Click this link to create or edit the checkbox options your visitors will be able to choose between on the actual form. You have the option to add each new checkbox to either the top or bottom of the list you are creating.

  • Label Placement - Choose one of the options.
  • Enable Spam Blocker - If the option "No" is selected, there is no spam blocker for the form. If the option "Yes" is selected, the spam blocker is on as a hidden form field.
  • Line Spacing - Increasing this will create space between each of your form fields. 10-15px is recommended.
  • Email address where notifications are sent - Enter the email address that will receive the form data when it is submitted. Enter multiple email addresses by separating them with a semicolon (;).
  • Submit Button Text - Change what the text on the Submit button says. It is typically recommended to leave as Submit unless that word choice doesn't fit well with your form.

Thank yous and Validations:
  • Online Thank You - You have the option to put a thank you message in this text box. If you do, then the website will show the visitor a page with your message on it after they click the Submit button on your form.
  • Email Thank You - Craft an email message to be sent after a form is submitted thanking the visitor for filling out your form. This is only an option if your form collects an email address.
  • Validation Email - You have the option to send an email to the visitor after they submit your form asking them to verify their email address by clicking on a link. This is only an option if your form collects an email address.
